Our fees are set according to the usual and customary charges in our area. If you have a question about fees, billing or insurance, you may call (414) 272-6955 during office hours and our patient account representative will assist you. You should also feel free to discuss our fees or billing procedures with your doctor.As a convenience to our patients we will file insurance forms to your insurance carrier. It is extremely important that we have accurate information if we are to bill your insurance for you. Please notify us promptly of any change in insurance coverage.

If you have a co-payment due on each visit the office, we request that the co-payment is paid at the time of service. If you have a traditional or PPO insurance, a monthly statement will be sent to you to show both the charges and payments posted to your account. You will be financially responsible for amounts not paid by your insurer.

Occasionally, unexpected financial circumstances may make it difficult to pay a large balance at one time. If this is the case, our patient account representative will work with you to make payment arrangements. At no time do we intend to let inability to pay interfere with your child’s medical care.

If you are covered under an HMO, you would generally not receive a monthly statement unless co-payments were not paid at the time of service.

Patients covered under a T-19 or Medicaid program must present a valid card for each visit.
